The Effect of Puzzle Play Therapy on Hospitalization Anxiety Among Preschool Children in the Pediatric Ward of Dr. M. Djamil General Hospital, Padang, 2026 Xiii + 73 pages, 4 tables, 2 figures, 17 appendices

ABSTRACT
Hospitalization is an experience that can cause anxiety in preschool children due to the unfamiliar hospital environment, medical procedures, and separation from their parents. The hospitalization rate among children increased from 1.88% in 2022 to 2.55% in 2023 and 2.99% in 2024. Hospitalization anxiety remains a common problem among preschool children. Data indicate that approximately 35–65% of children in Indonesia experience anxiety during hospitalization, while globally around 30–60% of preschool children experience anxiety while receiving hospital care. One non-pharmacological intervention that can be used to reduce anxiety is puzzle play therapy. This study aimed to determine the effect of puzzle play therapy on hospitalization anxiety among preschool children in the Pediatric Ward of RSUP Dr. M. Djamil Padang in 2026.
This study employed a quantitative method with a pre-experimental one-group pretest-posttest design. The study was conducted from March to August 2026, with data collection taking place from July 2 to July 11, 2026. The population consisted of 234 hospitalized preschool children, and a sample of 30 respondents was selected using purposive sampling. Data were collected using the Spence Children's Anxiety Scale (SCAS) questionnaire. Data were analyzed using univariate analysis and the Paired Sample t-test.
The results showed that the mean hospitalization anxiety score decreased from 58.80 before the intervention to 41.73 after puzzle play therapy, with a mean difference of 17.07. The Paired Sample t-test p-value of 0.000 (p < 0.05), indicating a significant effect of puzzle play therapy on reducing hospitalization anxiety among preschool children.
It can be concluded that puzzle play therapy is effective in reducing hospitalization anxiety among preschool children and can be applied as a non-pharmacological nursing intervention within the principles of atraumatic care to help children adapt to the hospital environment and reduce anxiety during hospitalization
